2. Visiting Medical Student Online Orientation, follow instructions, and print it.
3. Computer Access Forms.
4. Application for Visiting Osteopathic Medical Student.
5. Statement of Responsibility.
6. Complete, sign and mail or fax all forms (items 4-7) to:
